Welcome to SFEA online registration!

Currently, we are unable to accept online payments. You will need to follow up your registration by sending a check to our treasurer: Karen Cattell, 3576 N. Atherton St., Port Matilda, PA 16870. (You may also put your payment into the treasurer's box on the side of our mailbox unit at Park Forest Baptist Church.) NOTE: If you are a regular at SFEA and you know for sure that we already have your contact info on file, you may save time by writing "same" in the contact info boxes (unless your info has changed since last time you used this form, in which case it is vital to give us your current info!!). (Note: Please write in your name even if you put "same" in the other boxes. If you put "same" in for your name, we won't know who sent the email, Thanks!)

In this box, please list the student's name, the classes they will be taking during the quarter or semester you selected above, and the total payment for their classes during that time period. Example: Adam: Elementary science, Phys.Ed, art ($90) Eve: Middle school science, Reason to Write ($100) (Please erase all this text to type in your own.)
